Faten Mitchell Hassaan is the visionary behind, and lead advisor at Quality Improvement for Healthcare Inc (QI4H). She has over 10 years of experience aligning organizational strategic objectives that affect patient safety and family-centred care. She has successfully trained and mentored multidisciplinary and cross-sector healthcare leaders, providers, and inter-professional teams to build QI capacity and capability.

  • Reduced ED visits by 75% and admissions by 15% for patients living with COPD.

 
  • Designed, mentored, and hosted a Kaizen event for 70 people from six organizations to create a future-state cancer screening process.

  • Increased average cancer screening ratios: breast, 31%; cervical, 17%; and colorectal, 22%.

 
  • Analyzed six years’ worth of data from six hospital sites to determine causes of increased ED visits during Christmas and New Year, “holiday surge”. Findings include:

  1. No correlation between the number of ED visits and the day of the week on which Christmas and New Year fall.

  2. April and May had higher number of ED visits than December

  3. ‘Holiday Surge’ is related to reduced services between December 26 and January 1.
